All Players MUST HAVE

1. A Physical Completed and on File

 2. Insurance Waiver with Copies of Insurance Card and Parents Driver Lisence

or Have Paid $35 for HISD Insurance

3. All of the Athletic Paper Work Signed by Parent and Athlete

4.  All Freshmen and Transfer Students MUST have a PAPF completed and turned in
