The nurse is responsible for helping students with their health issues, medications, emergencies, and tracking the immunization status of the student body.Note from the Nurse:At DeBakey High School we are known for preparing students for careers in health care. It's my great pleasure to be of service to these wonderful students. My goal is to provide care for each student and maintain a healthy school environment. In order for me to do that, I need the parents' help. I ask each parent to notify the nurse of any medical problems a student may have and keep up-to-date immunization records. This will aid in providing efficient case management of your child(ren).

ALL 9th Grade students
Parents are to complete the Health Inventory and upload the immunization record
Students with AsthmaThe Asthma Action Plan is to be completed by your child's doctor. Your child must carry his/her own inhaler on them at all times.Students with Allergies and prescribed Epi-penThe Epi-Pen Form is to be completed by your child's doctor.Students with Food AllergiesThe "Physician's Request for Special Dietary Accomodation" is to be filled out by your child's doctor. (Medical diagnosis must be mentioned)Students who require any medication prescribed or over the counter medication during school hoursYour child's doctor will need to complete the "Request for Administration of Medication at School Form" and prescribe the medication including pain or any other medication. The medication is to be brought to the clinic in the original container as dispensed by the pharmacist.Please note that students may carry prescribed inhalers and EPI-PENs. Medications at School
Students requiring long-term medication or over the counter pain medication during school hours must have a Policies Governing Administration of Medication form completed by their physician and signed by a parent/guardian.The medication (even over the counter medication) is to be prescribed by the doctor and brought in by the parent issued by a pharmacist before the nurse can administer any medication.
To the Parents of Asthmatic Students:
The law allows a student with asthma to possess and self-administer prescription asthma medicine while on school property or at school-related events provided the following guidelines are met.
The following are the guidelines as per HISD standard practice memorandum and must be followed prior to a child possessing and carrying their inhalers at school.
A. Proof
The prescription asthma medicine has been prescribed for the student as indicated by the prescription label on the medicine.B. Administration
The self-administration is done in compliance with prescription or written instructions from the student's physician.
C. Parental Authorization
A parent of the student provides the school with written authorization for the student to self-administer prescription asthma medicine.
D. Physician
A parent of the student provides the school with a written statement form the student's physician, signed by the physician. The physician's note should clearly indicate the following information on the 40.3740 form (obtained from the school nurse or here.)
- The student has asthma and is capable of self-administering the prescribed asthma medication;
- The name and purpose of the medicine;
- The prescribed dosage for the medicine;
- The time at which or circumstances under which the medicine may be administered;
- The period for which the medicine is prescribed.

- Obtain a permit from the classroom teacher.
- Report to the clinic.
- The school nurse will contact the parent(s).
- Receive a permit from the nurse, and show the permit to teachers.
- Return to clinic with permit to:
- Wait to be picked up
- Sign out before leaving campus
Procedures for Leaving Campus for a Medical Appointment
- Bring a note concerning the appointment to the nurse by 8:00 a.m. on or before the day of the appointment. The note should include: the student's name, date and time of departure, classroom teacher at the time of dismissal, parent's phone number, and parent's signature. The nurse will call to verify the appointment with the parents.
- Pick up the permit from the nurse after 10:00 a.m.
- Show the permit to the classroom teacher and ask to leave the classroom at the appropriate time. (DO NOT wait to be called to the office by intercom). It is your responsibility to report to the clinic at the appropriate time.
- Sign out in the clinic before leaving campus
NOTE: If your parents are picking you up and signing you out for an appointment, you must follow the procedures listed above. A note must be on file in order to leave.
