Please note, when submitting your request, you will need to submit a signed physician's statement, on official letterhead, that clearly provides the following information:

• Your medical diagnosis/condition (severity and nature),
• The activities that are impacted or limited by your diagnosis/condition,
• The recommended accommodations, and
• How long the accommodations are needed.
