NEW STUDENTS - (Never attended YSAFE in 2019-2020) Please submit ; i) Proof of Residence * (Examples: utility bill (water, gas, light) Lease agreement or Mortgage receipt with current address) ii) Birth Certificate (or Birth Facts) iii) Proof of Income * (Examples: Last check stub, unemployment benefits, Social Security income, child support, etc.) iv) Social Security Card v) Parent/Legal Guardian Picture ID * (Examples: Driver License, State ID with Picture, Military ID, or Passport) vi) Immunization Records (for each child) vii) Report Card for students Grade 1 to 8 viii) STAAR Scores and IOWA Test Scores for 5th grade
