What if my child needs to take medication during school hours?
The medication administration form must be completed and signed by a healthcare provider and parent. Prescription medications and over-the-counter medications must be in the original container with a prescription label that contains the name of the student, medication name, dose, and frequency. Do not send the medication with your child. The parent needs to hand over the medication and form to the nurse.

What are the required immunizations before my child can start school?
- Dtap (5 doses): the last dose must be received on or after the 4th birthday.
- Polio (4 doses): the last dose must be received on or after the 4th birthday.
- MMR (2 doses)
- Hepatitis B (3 doses)
- Varicella (2 doses)
- Hepatitis A (2 doses)

What if my child requires a special diet?
Your primary healthcare provider will need to fill out the Special Dietary Accommodations form. Your physician may order to omit certain foods, may order texture of the food (soft, minced, pureed), and may order formula as a meal or supplemental. The form needs to be signed by the healthcare provider and parent. The form will then be forwarded to the district dietitian.
