7TH & 8TH - TUESDAY, OCTOBER 29 4:00-6:00
Girls trying out must have all of their paperwork completed and turned in to Coach Jackson before tryouts. No Exceptions.
PARENTS - GO TO UIL REGISTER MY ATHLETE AND SET UP AN ACCOUNT. PAPERWORK SHOULD BE PUT ON THIS WEBSITE.
1.physical on a HISD form
2. parental approval form
3. medical insurance - If an athlete does not have health insurance they must purchase HISD insurance for $35. If the athlete does have health insurance the parents are given the choice of purchasing HISD insurance which would be secondary insurance or they may sign a waiver stating they do not want HISD insurance. PROOF OF INSURANCE. THE WAIVER MUST BE NOTARIZED.
