    Paperwork will be completed entirely online on the Register My Athlete/AKTIVATE website. The links and all other information are located below. Players with health insurance who do not wish to purchase HISD insurance MUST complete the insurance waiver form and have it notarized. There is a notary located across the street at Parcels Plus for a small fee. Players without insurance MUST pay $35 dollars for HISD insurance through the SCHOOLPAY website. Please do not purchase the insurance on the RMA website. The physical, waiver or HISD insurance receipt must be scanned and uploaded to the RMA website.

    Next Level Urgent Care offers sports physicals for school aged children for $35.  All Next Level locations are open from 9am-9pm, 7 days a week and no appointment is needed to see our providers.
